We are currently accepting Letters of Interest (LOI) for our Community Benefit grant program. Our sixth round will award up to $500,000 in grant funds.

Letters of Interest are due no later than 4pm, Monday, August 21, 2017

Notice for Round 4 Grantees

If you are a grantee in the 4th round of our Community Benefit Grant Program, your six-month period ends on 6/30/2017, and your six month report is due no later than 7/31/17

About the 6 Month Progress Report

The 6 month progress report is intended to be a check-in on the status of your HCBF grant. This is an opportunity to share your progress on the grant to date, notify HCBF of any changes to the budget or scope, and discuss any unforeseen circumstances. The 12 month reporting form will ask for a more detailed account of your grant, including data collection and budget review. 

Please upload a Word document, single-spaced, 12 point font, of no more than one page, that addresses the following points: 
  1. Provide an overview of your grant activities in the first six months.
  2. List any adjustments made to the scope or budget, and why. 
  3. List any outside funding obtained as a result of this grant, or any new partnerships formed.
