This amended 12-month reporting form is for recipients of HCBF Community Benefit grants, Round 3. When crafting your report, consider building upon the six-month report you've submitted. 


The Grant period for this round began on 1/1/2016. This form is for grantees that were given an extension on the initial grant terms. Be sure to document any and all changes from the original grant. 


About the 12-Month Progress Report


The12-month progress report is intended to be the final status of your HCBF grant. This is an opportunity to share a more detailed account of your grant, including data collection and budget review.

Please upload a Word document, single-spaced, 12 point font, of no more than two pages, that addresses the following points: 

  1. Provide an overview of your grant activities for the full 12-months.
  2. List any adjustments made to the scope or budget, and why. IF the budget changed please provide a revised budget with actuals.
  3. List any outside funding obtained as a result of this grant, or any new partnerships formed.
  4. Please provide data for the outcomes of your project/program: qualitative and/or quantitative.

About the 6 Month Progress Report


The 6 month progress report is intended to be a check-in on the status of your HCBF grant. This is an opportunity to share your progress on the grant to date, notify HCBF of any changes to the budget or scope, and discuss any unforeseen circumstances. You can also share any pictures related to the HCBF grant. The 12-month reporting form will ask for a more detailed account of your grant, including data collection and budget review. 


Please upload a Word document, single-spaced, 12 point font, of no more than one page, that addresses the following points: 

  1. Provide an overview of your grant activities in the first six months.
  2. List any adjustments made to the scope or budget, and why. 
  3. List any outside funding obtained as a result of this grant, or any new partnerships formed.

If you report does not follow the requirements for the Word doc or does not contain all of the above requested information, it will not be accepted.
