About the 12-Month Progress Report
This progress report is intended to be the final status update of your HCBF grant. This is an opportunity to share a more detailed account of your grant, including data collection and budget review.
Please upload a Word document, single-spaced, 12 point font, of no more than two pages, that addresses the following points:
- Provide an overview of your grant activities for the full 12-months.
- List any adjustments made to the scope or budget, and why. IF the budget changed please provide a revised budget with actuals.
- List any outside funding obtained as a result of this grant, or any new partnerships formed.
- Please provide data for the outcomes of your project/program: qualitative and/or quantitative.
If you report does not follow the requirements for the Word doc or does not contain all of the above requested information, it will not be accepted. If you have questions, please contact HCBF at 310-997-7116 or email@example.com.
